Intake Forms and Important Information
Completing registration here after you have scheduled an appointment will provide us with the information, we will need to get you entered for services in our system prior to your appointment. After you have completed the registration process, click save and you will receive the basic intake forms on the next screen. Please complete the forms, print them (except for the Notice of Privacy Practices) and bring them with you to your first session. If you are unable to complete them prior to your appointment, please arrive 10 minutes early so that we can provide the forms to you to complete them in our office prior to your appointment.
If you are a new client seeking couples therapy each adult must register as a client and complete the appropriate forms.
